Do you think I was rude?

I had to pick something up for my dad at this place for grants. All I needed was one thing, I was on my lunch break so I didn't have a lot of time. I was VERY polite to the people when I walked in (I have been there before) I smiled and asked for the item I needed thanked the lady and before I could leave she handed me a paper on a clip board and told me that everyone has to fill one out. Last time I was there I didn't need to fill anything out and had been there a lot longer then just running in and out. I looked it over and started to fill it out, it was simple things like name address email etc... Then she tells me it was both sides. So I gave it back to her and told her I was sorry but I couldn't fill it out I was on a time restraint & left. I was real nice about it and she gave me a look like I violated the law or something! So was I rude? I mean I couldn't stay and fill out their questionere, which really since I wasn't there for me I don't see why I should have to fill it out!
